Anneke Rees and Tom Esakin (Facilitators) invite you to join us for an exchange of ideas, practical information and self-reflection around End of Life (EOL) matters. We will discuss how you and your loved ones can start to make choices and affect decisions around your own end of life and subsequent death. The knowledge learned can be applied to your life or when walking others through their dying process.

Anneke Rees holds 30 years of experience in the health care system, including as a palliative care paid-trainer and volunteer, and also on staff as a former Leader of Volunteer Resources for Providence Health Care – St. Paul’s Hospital. She is currently a member of the Canadian College of Health Leaders and a coach in their Executive Mentorship Program, and offers professional supports in hospice care.

Tom Esakin is formally-trained both as an interfaith spiritual director and in “Sacred Presence with the Dying”. He is the interfaith spiritual director at St. Paul’s Anglican Church Vancouver, offers spiritual supports through Reflective Presence-Consultants in Ministry, and is a member of SDI-Spiritual Directors International.
